Applications received in response to Advertisement No. SHS/NHM/J&K/ CPHC/AD/1799- 1806 dated 21-12-2020 were scrutinized for eligibility as per the criteria laid down in advertisement notice and accordingly the list of eligible candidates of Jammu Division and Kashmir Division is available on the official website of NHM, J&K www.jknhm.com along with the allotted Roll Numbers.
Written examination of eligible candidates for six (6) months Certificate Programme in Community Health for Mid Level Health Providers (MLHPs) shall be held on 24th February 2021, at 2:00 PM to 4:00 PM.
The eligible candidates shall submit the attached format duly filled by the candidate and collect Admit Card in person from National Health Mission office, Regional Institute of Health & Family Welfare, Nagrota, Jammu for Jammu Division and Ground Floor, Block- A, Old Secretariat Srinagar for Kashmir Division w.e.f. 13th February 2021 to 22nd February 2021 (10:00 AM to 4:00 PM). The format of Admit Card is also available on official website (www.jknhm.com) and at NHM, offices Srinagar/ Jammu.
All eligible candidates shall carry a valid photo identity (Passport, Adhar Card/ Driving Licence, Election Card) at the time of collection of Admit cum Roll no. slip.
No candidate shall be permitted to enter the Examination Centre without the Admit Card issued by NHM office.

Note:
1. Candidate shall carry the Admit Card to examination centre, failing which candidate will not be
allowed to sit in the Examination Centre.
2. Candidate shall carry Identity proof having photo graph (Aadhar Card, Driving Licence, Passport &
Election Card).
3. No electronic gadgets/ mobile phones will be allowed in the examination hall.
4. Candidates without Face Masks shall not be allowed in the examination hall.
